Pair Team coordinates medical, behavioral and social care for people on Medicaid and Medicare, connecting patients to doctors, housing, food and transportation through a dedicated care team backed by AI. The brand had to work on three audiences at once. The health plans buying the model, the community partners delivering it, and the members receiving the care.

We built it from scratch. New positioning, a full verbal and visual identity, and a new website launched on it.

Pair Team's model is now backed by a peer-reviewed study published in the Journal of General Internal Medicine: 52% fewer ER visits, 26% fewer hospitalizations, and over $20,000 in annual savings per patient. The approach is expanding to Medicare through the CMS ACCESS model, and Pair Team now works with more than 100 community partner organizations.
